This patient has undergone three previous procedures, including septoplasty. He essentially complains of a “destructured” nose, with a pinched tip (and inspiratory breathing difficulty) and a mild saddle deformity of the dorsum, with a certain feminization of the profile. Given the lack of sufficient material available from the septum, the procedure required harvesting a costal segment to restore the agreed dorsal height and correct the pinched tip, in addition to the use of DCF (Diced Cartilage in Fascia) to restore a slight convexity to the profile, giving back the distinctly masculine characteristics strongly desired by the patient.
