In this patient, who previously underwent closed rhinoplasty, there are evident gross asymmetries of the lines of the dorsum and the tip, which was resected inappropriately. The procedure will consist of meticulous soft-tissue dissection and identification of the residual structures, followed by an equally careful “reassembly” of these structures. For this purpose, as also occurs in primary rhinoplasty, a balanced approach will be required between resection (that is, reduction of the structures left in excess) and, above all, restoration (that is, reinstatement of the structures that were erroneously removed).

The aesthetic lines of the dorsum are now harmonious. The dorsum and tip have been reconstructed with new osteotomies and multiple grafts.
It was possible to correct the tip almost completely. The columellar scar is not visible.
In the three-quarter view, the proportions between dorsum and tip are now more harmonious.
The profile has been corrected with a graft at the radix, minimal controlled lowering of the residual dorsal hump, and reshaping of the tip.
