In this patient, the main characteristic is the bulbous tip, excessively voluminous and completely disproportionate relative to the dorsum. The alar cartilages are overly robust, overly convex, and “open,” clearly visible through the thin skin. The surgical plan for the tip involves reducing both volume and projection, as well as “transposing” the alar cartilages inferiorly to provide better support to the lateral rim of the nostrils.
