In this patient, the main characteristic is the bulbous tip, excessively voluminous and completely disproportionate relative to the dorsum. The alar cartilages are overly robust, overly convex, and “open,” clearly visible through the thin skin. The surgical plan for the tip involves reducing both volume and projection, as well as “transposing” the alar cartilages inferiorly to provide better support to the lateral rim of the nostrils.

The aesthetic lines of the dorsum have been improved by harmonizing the dorsum and the tip.
The bulbous tip has been corrected and “triangularized.” The access incision across the columella is barely visible. The aesthetic lines of the dorsum have been improved, creating harmony between dorsum and tip.
In the three-quarter view, the proportions between dorsum and tip have been corrected. The new structure of the alar cartilages—reduced and transposed inferiorly—creates a more aesthetically pleasing contour of the nostril rim, which now flows seamlessly into the tip.
In the profile view, the nose sits more proportionately within the facial framework, enhancing overall aesthetics. The dorsum transitions harmoniously into the tip.
