Primary

In the frontal view, the nose is deviated to the right and the aesthetic lines of the dorsum are asymmetric. The skin over the dorsum is thin. The tip is bulbous, almost square. The tip cartilages are robust and splayed apart, therefore too “open.” The two domes (the points where the alar cartilages of the tip curve toward the nasal alae) are clearly visible through the skin. In the profile and three-quarter views, the disproportion between the dorsum and the tip—overly robust and curved—is evident.

The aesthetic lines of the dorsum were improved by correcting the deviation and harmonizing the dorsum with the tip.
The square tip was corrected and triangularized. The transcolumellar access scar is minimally visible. Also minimally visible are the two small scars at the base of the nasal alae, where narrowing of the nostrils was required.
In the three-quarter view, the proportions between the dorsum and the tip were corrected, without creating an overly concave dorsum. The dorsum now transitions harmoniously into the tip.
The profile was improved with a radix graft, tip reshaping, and controlled lowering of the dorsum.
