The tip in this patient is nearly square. In the profile view, a moderate osteocartilaginous dorsal hump is visible. In the three-quarter view, the convex “bulges” on both sides of the tip are clearly evident, caused by excessive width and strength of the alar cartilages. Tip projection is adequate, and no radix grafts are required, as the nasal dorsum naturally begins at the level of the lash line.

The aesthetic lines of the dorsum were corrected by slightly narrowing the dorsum and, above all, by harmonizing the tip with the dorsum.
The square tip was corrected. The transcolumellar scar is not visible.
The dorsum, lowered and narrowed, now flows harmoniously into the tip.
The profile has been improved by lowering the dorsum and more clearly defining the transition to the tip (the “tip break”). The profile is straight, but slightly lower than the line connecting the radix to the point of maximal tip projection.
Postoperatively, the tip does not descend when the patient smiles.
Postoperatively, in the superior view, the dorsal aesthetic lines are harmonious.
